- Reduced PMPM medical costs by 7.25 percent

The HF Program produced the following results for a range of clients:

- Reduced hospital days by 17 percent

COPD Outcomes
The COPD Program produced the following results for a range of clients:
- Reduced emergency room visits by 23 percent and per member per month (PMPM) costs by 29 percent
- Reduced hospitalizations by 24 percent and PMPM hospitalization costs by 37 percent
- Reduced hospital days by 15 percent
Depression Outcomes
The Depression Program produced the following results for a range of clients:
- Reduced hospitalizations by 23 percent and per member per month (PMPM) costs by 15.25 percent
- Reduced emergency room visits by 11 percent and PMPM emergency room costs by 7.25 percent
- Reduced medical PMPM costs by 9.5 percent
- Reduced pharmaceutical PMPM costs by 7.25 percent
- Reduced total PMPM costs by 15 percent
- Clinical Performance Indicators:
- 89 percent of patients expressed satisfaction with the program
- 65 percent of patients said they improved previously unsuccessful relationships as a result of the program
Diabetes Outcomes
The Diabetes Program produced the following results for a range of clients:
- Reduced emergency room visits by 22 percent and per member per month (PMPM) costs by 14 percent
- Reduced hospitalizations by 28 percent and PMPM costs by 27 percent
- Reduced hospital days by 16 percent
- Reduced pharmaceutical PMPM costs by 9.75 percent
- Reduced total PMPM costs by 9 percent
- Clinical Performance Indicators:
- Increased at-home blood-glucose testing by 21 percent*
- Increased LDL screenings by 16 percent
- Increased urine testing by 48 percent
- Increased eye exams by 11 percent
*Only 2 percent failed to reach nationally acceptable levels of blood-glucose testing.
Weight Management Outcomes
The Weight Management Program has produced the following results for a range of clients:
- Reduced weight for participants of 180 days or greater by 10.5 percent
- Improved presenteeism at work by 20 percent
- Reduced absenteeism by .5 percent
- Increased activities outside of work by 15 percent
