One of the keys to Matria’s success is offering the appropriate interventions based on an individual’s risk factors. While Matria’s Disease Management programs address those with medical conditions and the online Healthy Living Programs cover a wide range of users with low-to medium-risk, the Health Coaching program targets high-risk users with a cost-effective telephonic intervention.
How Health Coaching Works
When a user’s Health and Productivity Assessment indicates an elevated risk, that individual may be eligible for Health Coaching. Coaching is offered in the following categories:
- Weight Loss
- Smoke-Free
- Get In Shape
- Stress Relief
- Healthy Heart
- Diabetes-Fighting
Participants enrolled in Health Coaching receive printed booklets highlighting key elements of the program, and get access to the online Healthy Living Programs.
Expert Care to Reduce Risk
The program is based on best practices of behavior-change management. Coaches help participants set goals and provide the tools and resources to support sustainable changes in behavior. The approach is consistent across all modalities, with the online and print materials supporting the telephonic intervention. This helps users learn to maintain their healthier behaviors independently.

- Registered Dietitians
- Registered Nurses

- Exercise Physiologists
- Master’s level Social Workers with expertise in counseling
Eligibility
Eligibility for Health Coaching is determined based on the following risk criteria from a user’s Health and Productivity Assessment:
- Cholesterol level and blood pressure
- Body Mass Index
- Smoking frequency
- High or extreme stress level
- Low physical activity
- A combination of measures including age, family history, body mass index, blood pressure and lab values
